SMS Spring Sports Information 2023-24 School Year

*Please register for all Fall sports through RankOne

Registration and Required Forms are below and can be accessed through Rank One:

• Athletic Clearance Forms

• Physical Form (Must be on file at school. Good for two years from appointment date) Send to Cyelvington@scappoose.k12.or.us or drop off at SMS office (bbarnard@scappoose.k12.or.us)  

• Physical Form- Spanish (Debe estar archivado en la escuela. Válido por dos años a partir de la fecha de la cita) Enviar a Cyelvington@scappoose.k12.or.us o dejar en la oficina de SMS
